PLAY TO PARTICIPATE FEE:
All N.H.S. student athletes who become a member of a team, must submit a non-refundable Pay to Participate Fee for each sports season ($400 Family Cap per year)
once the final teams have been selected.
PLEASE PAY THE PLAY TO PARTICIPATE FEE ONLINE VIA THE MY SCHOOL BUCKS LINK ON FAMILY I.D. WHICH IS FAST AND EASY! You may sign up with My School Bucks, or make your payment as a guest. The PTP fee is used to help off-set the cost of the sport, and in no way guarantees a set amount of playing time

CONSENT TO TREAT:Each parent/guardian is required to complete an online Consent to Treat form indicating pertinent medical information, emergency phone numbers and a contact person in case of accident or injury. These Consent to Treat forms are kept confidential in the first aid kit, and accompany teams to all practices and games. It is the parents/guardians responsibility to contact the coach and revise this information if it changes during the course of the season/year.
PHYSICAL FORM / PARENT PERMISSION FORM:
It is the policy of the N.H.S. Athletic Department, that all parents/guardians submit an updated physical examination form for their Student Athletes, directly to the Nurses Office before they are allowed to try out for a team. The physical must be documented on the blue health form which is good for thirteen months from the date of the physician's signature. It is a good idea to have the physical examination during the summer so that the form is valid for all three sport seasons.
In addition, all parents/guardians are required to sign off on the online Parent Permission Form.
