Welcome to Jefferson High School's online-only sports registration. This system provides convenience and efficiency for you, the coaches and the athletic office. After you sign up once, it's very easy to register for other sports or add siblings.
This form is to be filled out by the parent/guardian and athlete together.
Fall sports practices begin August 16th. Completing this online registration and having a current physical form on file is required before an athlete can practice or participate in competitions.
OSAA rules mandate that all student athletes have a physical examination every two years and turn in a specific form. If the athlete has not had a physical within the past two years or the physical is expiring, please schedule an exam and have your medical provider complete the required physical exam form (found in the links section on the right hand side of this page). We also have these forms in the Athletic Office. By Oregon law we need a hard copy of the completed physical form. You can upload your physical form to Familyid, submit forms to the Athletic Office or email them to Falisha Wright at email@example.com. Families should keep a copy of the physical.
PPS does not provide medical insurance for Athletics. If low-cost Student Health Insurance Coverage is needed, please see links to right.
Participation fees are used to offset the expense of coaching stipends.
We acknowledge that this is a difficult time for families, and we do not want money to ever be a barrier to participation.

You can pay the PIL sports fee through School Pay. If an athlete needs to quit a sport due to injury or academics three weeks or less into the season, a partial refund will be arranged.
