Before any student can participate in tryouts, the following requirements must be met:
• All students MUST turn in a copy of their most recent physical to the athletic office prior to the first day of practice. Physical exams are valid for 12 months from your last annual exam. Students with expired physicals will not be permitted to participate in practices or games until a new physical is turned in.
• The parents/guardians are responsible for filling out this form completely and provide their electronic signature prior to submitting their sports registration through the Manchester School District. In completing the Sports Registration, via Family ID, the parent and/or guardian ensures that any and all medical information concerning their child's health, medical concerns, injuries, medications and allergies are updated and accurate as reported to the Athletic Department & their respective coaches.
All students must pre-register, be cleared physically fit to play by a licensed physician, must be academically eligible, and have demonstrated satisfactory citizenship, both in their school and community, prior to participating in their respective sport.
In response to the COVID-19 pandemic, baseline concussion testing procedures have been modified for fall sport athletes. For Fall 2020, only incoming 9th grade and new contact and collision sport (field hockey, soccer, and football) and cheer athletes must complete a baseline C3 Logix (iPad) assessment with Safe Sports Network. Returning contact and collision sport athletes who have previously completed a baseline C3 Logix assessment with the Safe Sports Network will not need an updated C3 Logix assessment. If fall sports resume as originally planned, baseline concussion assessment scheduling with Safe Sports Network will open in August. Scheduling information will be shared with coaches to share with athletes once open.
