1. Each student athlete must have documentation from a doctor that he or she has had a physical exam within the last thirteen (13) months. Physical exams are no longer valid after thirteen months to the day of the last physical. This information is located on the MIAA website (MIAA Medical Questionnaire & Physical Form)
Please note that if your child has one on file with the school already, the date should be verified with either the Athletic Department's staff-medic or the Nurses Office. However, a copy should be given to the Athletic Department to ensure the paperwork is in order. We are very fortunate to be able to track this data through the FamilyID database.
***A new feature added to the registration process allows you to upload a copy of the current physical form to have on file for your student/athlete.
2. A parent/guardian must fill out the Family-ID form completely.
3. Students who plan to participate in the following sports are required to take the baseline IMPACT test.
- Field Hockey
- Boys Soccer
- Girls Soccer
This test can be done from your home, and will need to be completed before the student can participate. CLICK HERE FOR MORE INFORMATION
Students are only required to take the test once during the school year unless they sustain a head injury.
4. YOU CAN REGISTER WITHOUT PAYING THE USER FEE.
*All User Fees must be submitted to the Main Office by September 1, 2020

Any questions regarding financial obligations must be directed to the Athletic Department prior to the start of tryouts.
