2020 Fall Season Sports Sign-up & Registration Information
High School Fall Sports: Cheerleading, Cross Country (Boys & Girls), Dance, Field Hockey, Football, Gymnastics, Soccer (Boys & Girls), Girls Tennis, Girls Volleyball
Upper Middle School Fall Sports (7th & 8th grades only): Cross Country (Boys & Girls), Field Hockey, Soccer (Boys and Girls)
For your child to be cleared to participate on an athletic team, the 3-step process detailed below must be completed. All team managers and statisticians need to complete steps 1 and 2 only.
**Important Information for Fall 2020 Sports: Registration is now open in Family ID. However, we will not be accepting any sports physical forms at this time. If you schedule a sports physical with your physician, please hold on to the forms until we communicate information on how we will be collecting these. In addition, registration for Fall Sports is contingent on the reopening of schools as well any additional guidelines set forth by our governing health and athletic associations.
Step 1 - Register Online with FamilyID:
The Montgomery Athletic Department uses the FamilyID athletic registration program. You must register for a team each sports season. Failure to register each season with FamilyID will prevent your child from being cleared to participate. All sports registrations must be completed by a parent or legal guardian. Students are not permitted to register on FamilyID. The FamilyID program does not include the submission of the Physical Evaluation Form, the Health History Update Form or the Allergy Action Plan.
FamilyID sports registration for the 2020 Fall season will open on May 22nd at 12:00pm. All FamilyID registration must be completed by August 17th for all MHS athletic programs and August 31st for all UMS athletic programs.

Step 2 – Pre-participation Physical Evaluation – HISTORY FORM & PHYSICAL EXAM:
All student athletes need to have an active sports physical, recorded on the required form located on our Athletic Dept. website under FORMS. Sports physicals are valid for 365 days and need to be active on the official NJSIAA start date for the respective sport being played.
All Physical Evaluation Forms must be reviewed by our medical staff and our school physician per NJ state law for your child to be cleared to participate.Please keep in mind that after we receive your sports physical, it needs to be reviewed by our medical staff for completion, then submitted to our school physician's office to be reviewed a final time and signed off. DO NOT WAIT UNTIL YOUR SEASON STARTS!!!
***Physicals should be turned in at least 3 weeks prior to the start of respective sports season***
Forms are not to be handed into the main office, emailed, faxed or mailed to the school at any time. We will only accept an original document. The health office will deliver the forms to the school physician for review.
The Allergy Action Plan Form is required once per year ONLY if your child’s physician has prescribed an Epi Pen for your child’s allergy. The allergy action plan must be renewed by your physician each school year. Please note that your child will not be allowed to participate in athletics without a completed and updated Allergy Action Plan on file in the nurse’s office.

Step 3 - ImPACT Baseline Test (MHS Sports only):
The online ImPACT baseline test is required once a year during your freshman & junior years before the athlete’s first sport. If you are going to be in 9th (Class of 2024) or 11th grade (Class of 2022) or you are new to Montgomery High School, you will need to take the baseline test. If you are currently being treated by a doctor for a concussion, do not take the baseline test and see one of the Athletic Trainers ASAP.
