THE REGISTRATION DEADLINE FOR WINTER SPORTS IS JANUARY 19, 2021

THE PAY TO PARTICIPATE FEES ARE BEING DEFERRED UNTIL FURTHER NOTICE.
PLEASE DO NOT PAY NOW WITH YOUR REGISTRATION. WE WILL SEND OUT A NOTIFICATION AS TO WHEN WE WILL BE COLLECTING THE WINTER ONLINE PTP FEE. WHEN PAYMENTS ARE DUE, YOU WILL NEED TO PAY VIA MY SCHOOL BUCKS, WHICH IS LINKED TO YOUR PERSONAL FAMILY ID ACCOUNT.
IF YOUR STUDENTS'S PHYSICAL IS NOT CURRENT, A UPDATED ONE MUST BE UPLOADED ON YOUR FAMILY ID ACCOUNT. SHOULD YOU HAVE ANY QUESTIONS, PLEASE CALL THE NURSES OFFICE DIRECT - (203)426-7649.
Consent to Treat Form: Each parent/guardian is required to complete a consent to treat form indicating pertinent medical information, emergency phone numbers and a contact person in case of accident or injury. These consent to treat forms are kept in the first aid kit and accompany teams to all practices and games. It is the parent's responsibility to contact the coach and revise this information if it changes during the course of the season/year.
Physicals: It is the policy of the N.H.S. Athletic Department, that all student athletes physical forms be uploaded to Family ID. These uploaded forms are sent directly to the Nurses Office, as all N.H.S. athletes must have a current physical examination form on file before they can try out for a sport. The physical date must be documented on the health form and is valid for fifteen months from the date of the health visit. It is a good idea to have the physical examination during the summer so that the form is valid for all three sport seasons.
Pay for Participation: All members of N.H.S. teams must submit an online NON-REFUNDABLE Pay to Participate fee for the sports season, once the final teams are selected. This fee is used to help offset the cost of the sport and in no way guarantees a set amount of playing time. There is a $450 family cap per year.
