Welcome to Lakeview High School Athletics. Lakeview High School is excited to once again be using FamilyID for online sports registration for our families and players - no more paperwork!
All parents/guardians and their student-athletes must complete the following information to gain clearance before the first day or tryouts/practice. Only cleared players are eligible to participate. Electronic signatures are considered legally binding and must be completed by the student-athlete and by a parent/guardian over the age of 18.
A current physical exam dated on or after April 15, 2020, must be fully completed prior to participation in an athletic practice. All physicals must be on the MHSAA physical form. No other forms will be accepted.
For the 2020-21 school year, the MHSAA has allowed for parents to fill out a sports health questionnaire in place of a current athletic physical. This Sports Health Questionnaire may only be used for students who received a valid sports physical during the 2019-20 school year (one completed on or after April 15, 2019). A copy of the 2019-20 sports physical must be on file along with the questionnaire in order to be eligible. Lakeview Public Schools encourages a current sports physical (dated after April 15, 2020) to be used as the questionnaire does not take the place of a physical.
