Before beginning SUMMER pre-season practice (beginning July 1st, 2020) or official practice beginning Nov. 20th WINTER athletes MUST:
1) Register for their sport on the Family ID website.
2) Turn in to the Athletic Office an updated sports physical (ONE PAGE only - Section 6) dated June 1, 2020 or after. This Section 6 page is available to print by opening this link: https://1.cdn.edl.io/StPutIbKc... OR is available in the school main office. The rest of the physical form is now completed on the Family ID website. After you complete the health history section (51 questions) please make sure to print the health history section and take it with you to your physical - the doctor will need to review the questions during the exam. Please see your own doctor or go to a faster care clinic for your child's physical - physicals are not scheduled at school this summer.
Athletes who already played a 2020 fall sport and turned in a physical DON'T NEED TO TURN IN ANOTHER PHYSICAL. One physical per year.
Before official practice begins (Nov. 20th) WINTER athletes must also:
Have a completed *baseline Impact test (concussion test) on file at the school that is less than 2 years old. Students in grades 7, 9, and 11 must be tested before the first day of practice. Testing dates/times will be announced later for winter sports. *While we realize that your child may have taken the baseline test at CCP or your personal physician's office, we request that your child also take the test at Armstrong Jr/Sr High School so that we have the information readily available.
Returning athletes must complete the re-certification questions at the end of the 51 Concerns and Comments questions.
The $30 participation fee is due by Nov. 28th and should be turned in to the Athletic Office (only NEW athletes - athletes that paid in the fall don't have to pay again)
BOWLING and HOCKEY registration are not on this website (not school sponsored sports).
