2020-2021 Required Health Forms

Dear Tower Families,

Thank you for taking the time to fill out the required health information.  Due to Covid-19, we ask that all information is provided and forms are received by July 15th.  If you have any questions or need any assistance, please do not hesitate to contact me at paula.cardarelli@towerschool.org.

Returning Families:
Please update and provide any new information regarding your child's/children's health.  All students are required to have a yearly physical on file prior to the start of school. If your child is entering Pre-K, Kindergarten, or Seventh grade, we also need a copy of an updated immunization record.  If your child/children are not immunized due to a religious exemption, a letter must be written and signed by parents and/or guardians and dated yearly.  Medical exemptions must be filled out by your child's/children's pediatrician.
If your child has asthma, allergies, or any health condition that requires them to take medication during the school day, please download, mail, or email me a copy of your child's action plans.  Children with asthma require an Asthma Action Plan on file at school.  Children with allergies that require an epi-pen require an Allergy Action Plan on file at school.  These forms are provided by your child's pediatrician or their asthma and allergy specialist.

New Families:
