**Covid 19 Registration protocol for Fall 2020 Sports
No student shall represent their school in inter-school athletics until this registration form is on file with the Athletic Department as well as a CURRENT, signed physical form certifying that he/she has passed an adequate face to face physical examination WITHIN THE PAST YEAR, noting that in the opinion of the examining physician, physician's assistant, nurse practitioner or a certified/registered chiropractor, is physically fit to participate in high school athletics; that student has the consent of his/her parents or legal guardian to participate; and, the parent and participant have read, understand and agree to the CHSAA guidelines for eligibility. Physicals must be uploaded on this registration form or faxed to 303-655-8920. In-person delivery of physicals will not be accepted at this time.
An Athletic fee of $135 ($195.00 for non SD27J students) is also required in full before any athlete can participate in practice/tryouts, as well as all student fees paid in full per District 27J Policy. Payment of all fees (Athletic, Course, Transportation) will be accepted ONLY online on the District 27j website ($ Student fee payments tab). Check/Cash payments will not be accepted at this time.
**NOTE: There will be a 24 hour window before you are cleared to participate in a practice.**
FEE WAIVERS (FREE OR REDUCED LUNCH)
Applications are available through the Nutrition Service Department by clicking on the following link http://www.sd27j.org/Page/294 . Please be sure to check all boxes in Step 5 to insure your fees are waived. Any questions, you may call - 303.655.2986.
If you are a transfer student from another high school, played sports for another high school, or are an exchange student you will be required to notify the athletic office for instructions on how to complete additional information electronically through Planet High School.
HOMESCHOOL/ONLINE/ATTENDING ANOTHER SCHOOL
When turning in your athletic paperwork, please include your last semester grades, contact information for attendance confirmation and proof of residency. (Note: your address of residency will determine the school you should attend for athletics.) Please see CHSAA Form 6 link to your right.
ELIGIBILITY FOR PARTICIPATION
PVHS follows CHSAA Eligibility Plan A Requirements: Students must be enrolled in a minimum of 5 classes to participate. Students enrolled in the minimum requirement of 5 classes and must be passing those five classes. See tab for additional 27J eligibility information.
