ATHLETIC PHYSICAL EXAMINATIONS
The athletic department will offer physical examinations at the end of every school year. Students not participating in these physicals must have physical examinations by their own physicians before any practice will be allowed or uniforms issued.
Sports physicals are only good for one calendar year. Students must have a current physical and completed registration on file before trying out for any sport.
Sport Physicals will only be accepted on the provided physical form. You can get the form under the "Policies" and "Links" sections in your FamilyID registration, on the school website under Athletics/Resources and in the school's front office or Athletic's office. Thank you for your help in this effort to make athletic registration more efficient.
NO EQUIPMENT FOR ANY SPORT WILL BE ISSUED UNTIL EACH PROSPECTIVE ATHLETE HAS COMPLETED A PHYSICAL AND REGISTRATION, PARENT PERMISSION, EMERGENCY PROCEDURE CARD, INSURANCE VERIFICATION AND ACADEMIC ELIGIBILITY.
