2020/2021 Athletic Registration

ELIGIBILITY REQUIREMENTS:

1. Current ASB card ($15)

2. Must pass a physical exam from a licensed physician. Physicals are good for 24 months.

3. Complete this FamilyID registration form

4. Carry a minimum of a 2.0 grade point average and be passing five out of six classes during the previous quarter.

5. Read and agree to "Safety Guidelines" for each sport the student-athlete wishes to participate in and sign up for all the sports and sport seasons.
