1. Each student athlete must have documentation from a doctor that he or she has had a physical exam within the last thirteen (13) months. Physical exams are no longer valid after thirteen months to the day of the last physical. This information is located on the MIAA website (MIAA Medical Questionnaire & Physical Form)
Please note that if your student athlete has one on file with the school already, the date should be verified with either the Certified Athletic Trainer or the Nurses Office. However, a copy should be given to the trainer to ensure the paperwork is in order.
***A new feature added to the registration process allows you to upload a copy of the current physical form to have on file for your student/athlete.
2. A parent/guardian must fill out the Family-ID form completely.
3. Students who plan to participate in the following contact sports (Football, Boys Soccer, Girls Soccer, Field Hockey, Cheer and Volleyball) are required to take the baseline IMPACT test. This test can be done from your home, and will need to be completed before the student can participate.
Students are only required to take the test once during the school year unless they sustain an injury. For example, a student who participates in a Fall sport would not be required to take the test in the Winter or Spring.
4. User Fee payments are encouraged to be made via PayPal at the time of registering.
If necessary, checks can be written out to "NM Athletics" in the amount listed after the registration process is completed. The fee must be submitted prior to tryouts and no later than the first day of practice.
***We will hold all checks until the roster is completed by the coaches. The fee is non-refundable after the first 10 days of the sport.
Any questions regarding financial obligations must be directed to the Athletic Department prior to the start of tryouts.
