STEPS FOR ATHLETIC CLEARANCE
The athletic office must clear students-athletes, in order to participate in practices and contests. Please allow 48 hours for the clearance process.
The student must complete the following procedures EACH SEASON.
1. PAY PARTICIPATION FEES -
Students must go to the bookkeeping office to pay their participation fee. All outstanding fines must be paid before the bookkeeper can process participation fees.
The cost to participate in athletics is $200 per sport with a $600 annual family cap. Individual students have a cap of $400. Please be sure to keep all receipts as proof of payment. Refunds will be issued or prorated if an athlete is cut from a program or if they suffer a season ending injury. Refunds will not be given if an athlete quits voluntarily or is dismissed due to disciplinary reasons. Please contact the bookkeeper for refunds or questions at 503-565-4101.
2. Physical Examination
Students must have on file a record of a current physical examination that has been completed on the state-approved physical examination form (in the main office or the admin office). The physical must be dated AFTER July 1 of the students freshman year. Only the state-approved physical examination form will be accepted.
3. Impact Testing

4. Complete Athletic Participation Below
Students must complete the registration form below and it's to be signed by both the parent/guardian and the student. Be sure to read the Code of Conduct prior to signing the form.

5. Proof of insurance
Students must have proof of insurance on file in the athletic office. Families who do not have insurance may purchase insurance from Myers-Stevens & Toohey & Co. They offer several options ranging in price. Forms are available in the main office or the admin office.
6. Meet Eligibility Standards
Any student who has passed five classes the previous semester and is currently enrolled in five classes is ELIGIBLE to participate in after school athletics and activities as set by OSAA standards. Additionally, a senior must start the year with a minimum of 19 credits, a junior 11 credits and a sophomore 5 credits.
Any athlete who has one or more F's OR has a GPA of less than 2.0 from the previous semester will be deemed on Academic Probation and must follow procedures outlined in the Academic Probation Protocol.
Students who are placed on Academic Probation and are participating in athletics or a club/activity may continue to do so with the agreement that the student will attend mandatory study halls. The student must attend the study halls for the remainder of the grading period. If the student does not fulfill the required time in the study hall each week, they will not be able to participate until the hours have been completed.
At the end of the grading period the student must demonstrate that he/she is passing all classes and have a GPA of higher than 2.0. If the student is passing all classes and has a GPA of 2.0 or higher, then the student will no longer be on probation and will not be required to attend the study halls.
If at the grading period the student is still not passing all classes and/or the students' GPA is below a 2.0, then the student will immediately be declared ineligible and will not be able to participate in athletics until he/she is passing all classes and is earning a GPA of 2.0 or higher.
Students need to be aware that at any time their eligibility in athletics and clubs/activities may be revoked due to unsatisfactory attendance or academic performance.
Ineligible student-athletes may obtain permission from their coach to be able to practice with their teams but under no circumstances will they be allowed to dress down or participate in any games.
Students who are placed on academic probation must follow the procedures of the Academic Probation Protocol in order to participate.
Students are cleared to participate once all of the above steps have been completed and verified. Only the athletic office personnel can declare a student-athlete cleared to participate in athletics. Upon completion of all of the above steps, the athletic office will forward a copy of the Athletic Participation form to the coach which will serve as notice as to whether or not a student is eligible to participate.
