USER FEE PAYMENT: Payment is expected by the first day of all Activities. Only checks or money orders are accepted if payment is not done online. User Fee checks and money orders must be made payable to the“North Attleboro School Department".

PERMISSION SLIP: The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Advisors.
