To be eligible for athletics, a student must:
*Have a current pre-participation physical exam. Pre-participation physicals are good for 395 calendar days and all athletes must have a current physical on file before he/she can participate in tryouts/sports at MHS.
* Provide proof of insurance on this form– either through personal/family plan or school purchased plan.
* Have passed 5 out of 7 units for previous semester (year-long classes count as 1 unit; semester classes count as 2 units)
* If taking less than a full schedule students must still pass equivalent to 5 units (year long as 1 unit; semester class count as 2 units)
INSTRUCTIONS: Please complete student information section and be sure to include current Insurance Policy number or Group number. Please thoroughly read through our school Policies which include the "Dare County Schools Drug Testing Policy" "Dare County Schools Discipline Procedures for Student-Athletes" "Gfeller-Waller Concussion Information for Student-Athletes & Parents/Legal Custodians" and the "2019-2020 North Carolina High School Athletic Association Eligibility, Consent to Participate and Release Form". Please read carefully and be sure the correct person is signing in each box. The Student-Athlete must sign in the box that is titled "Student," and the Parent or Legal Guardian must sign in the box titled "Parent/Guardian - Signatory must be older than 18".
