The first day of the 2019-20 school year is Tuesday, August 27, 2019.
To assist us in getting everything in order for that first day, please provide information for each child who will be attending Lyme-Old Lyme High School this year.
STUDENT DEMOGRAPHIC INFORMATION
As part of this back-to-school process, be sure to enter your own contact information, as well as that of adults you authorize to be emergency contacts for your child.
Our school office staff will check the demographic information you provide on this page against what is stored in our PowerSchool student information system, and input changes.
Phone numbers stored in PowerSchool are used by School Messenger, our automated messaging system, to call telephones or send text alerts in the event of school closings/early dismissals/important notices.
HEALTH AND MEDICAL INFORMATION
All health and medical information that you provide on this page will be given directly to the school nurses to enter into their confidential records.
AGREEMENTS TO SIGN
Please sign any agreements listed at the end of this page, including the one giving permission to treat your child in the event of a medical emergency.
For some agreements, your student's signature is also required.
