As you know, we have incorporated FamilyID as a web-based collection of information. Parents and the students will again need to utilize their FamilyID accounts or if needed, create an account on FamilyID.
WHEN FILLING OUT THE REGISTRATION...PLEASE BE SURE TO HAVE YOUR STUDENT REVIEW THE AGREEMENTS LISTED BELOW AND ENTER THE STUDENT NAME IN THE AREA MARKED "STUDENT". PLEASE DO NOT LIST THE PARENT NAME IN THE "STUDENT" AREA. YOU WILL BE SENT AN AGREEMENT CORRECTION REQUEST AND YOUR STUDENT WILL NOT BE CLEARED FOR ATHLETIC PARTICIPATION UNTIL IT IS CORRECTED.

WHEN YOUR STUDENT PLANS TO SEE THEIR PERSONAL PHYSICIAN, YOU WILL NEED TO PRINT AND TAKE SECTION 6:PIAA FORM : ***Physical must be completed on or after June 1, 2019*** If your student is unable to see their Personal Physician, a CIPPE athletic physical can be obtained at MedExpress for $30.00. Please ensure that Mrs. Lisa Popovich, the High School Nurse, receives a copy of your completed SECTION 6: PIAA COMPREHENSIVE INITIAL PRE-PARTICIPATION PHYSICAL EVALUATION (CIPPE) AND CERTIFICATION OF AUTHORIZED MEDICAL EXAMINER. This can be done by emailing a scanned or clear photo of the completed Section 6 form to firstname.lastname@example.org , faxing to 724-259-6476 or dropping off the form at the high school office.
If your PCP requires you to provided them with a completed Section 5: Health History & Section 6: PIAA Initial Pre-Participation Physical Evaluation, please print the form from the link. We no longer use this form as it is incorporated into our FamilyID registration.
Baseline Concussion Tests must be completed with the PTHS Athletic Trainers prior to full clearance being received. If your student has a completed ImPACT test with their physician within the last 2 years, you may submit that as a baseline. If your student has already completed a Baseline Concussion Test, they do not need a new one unless they have had an injury (i.e. concussion) or are informed by the PTHS Medical Staff.
For Baseline Concussion Testing information:
During the school year, Baseline Concussion Tests can be obtained in the High School Athletic Training Room, Monday through Thursday from 2:30-4:00pm. If you are unable to attend during that time, please call 724-744-0580 to schedule with one of the Athletic Trainers.
If you are submitting your registration after the July 29th deadline, please allow up 4 days for all registrations and CIPPE forms to be cleared after they are submitted to the High School Nurse; FINAL CLEARANCE WILL BE GIVEN BY HS MEDICAL STAFF. It is highly recommended that registrations are completed and CIPPE forms are turned in at least two weeks prior to the start of fall sports (August 12th). Registrations and CIPPE forms submitted late will not be guaranteed the opportunity to participate on the first day of practice/tryouts. ALL registrations and/or CIPPE Sections 6 forms will NOT be cleared the same day they are submitted to the High School Nurse.
You can always check the status of your child's Athletic Participation Clearance Status by logging into your FamilyID Account and selecting "VIEW OR EDIT MY REGISTRATIONS" select the "SUMMARY" tab at the end of the current registration.
You will see a blue section that says "MEMO FROM PENN-TRAFFORD ATHLETICS".
The information in that section will tell you the status of their clearance for athletic participation.
Schedule of PIAA start dates for FALL Sports and DUE DATES for completed athletic physical registration:
July 29, 2019 - Last day to register on FamilyID and submit Section 6 (CIPPE) & for ALL varsity, junior varsity, junior high and middle school fall sports teams.
August 5, 2019 - First day High School Football Heat Acclimation.
August 12, 2019 - First day of Fall Varsity Sports.
