Before any student can participate in tryouts, the following requirements must be met:
• All students MUST turn in a copy of their most recent physical to the athletic office prior to the first day of practice. Physical exams are valid for 12 months from your last annual exam. Students with expired physicals will not be permitted to participate in practices or games until a new physical is turned in.
• The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Athletic Department & coaches.
Please Note: All students must be cleared physically and be academically eligible prior to their participation in their their respective sport, for that season.
BASELINE CONCUSSION TESTING
For the 2017-2018 school year, all contact sport athletes will be required to have a valid (within 2 years) ImPACT test (computer version) and our new C3 Logix (iPad) baseline test on file. ALL contact athletes must complete the new C3 Logix testing. Freshman and Juniors must ALSO complete the ImPACT test. Check with your school's athletic trainer for testing dates. Additional dates are available at the Safe Sports free injury drop-in clinic. Please call their office at 627-9728 to set up an appointment.
