I. Physical Examinations
The Colorado High School Activities Association requires that each athlete complete a physical examination prior to participation in practices or games. The physical is good for a 365-day period. The physical exam is to be conducted by a physician, nurse practitioner, physicianʼs assistant, or Doctor of Chiropractics who is School Physical Certified (DC, Spc.) of your choice. A "Physical Examination" form is available from all local clinics. If a physician outside of the school district will give the physical, forms are available at the high school and district office. This form contains two parts. One part is to be completed by the examining physician. The other part is to be completed by the parent(s) or guardian(s) granting permission to play. This form must be complete before the prospective athlete may check out equipment or participate in any practice sessions. If the examining physician places any restrictions on the athlete with respect to certain sports, taping, medication, or other requirements, those need to be written on the back of the physical form.

II. Insurance
The East Grand School District requires that every athlete, student trainer, and manager be covered by personal or group health insurance before the athlete can check out equipment and participate in any practice sessions. If a family does not carry insurance, the school has information about an insurance policy that may be purchased through the school.
