Medical Requirement

The New Jersey Department of Education, in accordance with the Scholastic Student Athlete Safety Act, has implemented a revised Pre-participation Physical Evaluation Form (PPE) which is to be utilized by all schools beginning May 2014.  THIS IS THE ONLY ACCEPTABLE FORM FOR ATHLETIC MEDICAL CLEARANCE!  The PPE form includes a History section, to be completed and signed by the parent/guardian; a supplemental history form for students with special needs (if applicable); and a physical examination section and clearance form to be completed by the examining home physician, schools physician, advanced practice nurse (APN) or physician assistant (PA).  Please note that Teaneck High School requires your doctor to sign and stamp all physical forms.

In addition to the PPE forms, student-athletes and their parent/guardian are required to review and acknowledge the following documents:
Random Drug Testing for Banned Substances
Concussion Policy Acknowledgement Form
Sudden Cardiac Death in Young Athletes Pamphlet
Lastly, each student-athlete and their parent/guardian are required to complete and execute the Athletic Registration Card.
