2018 Fall High School Athletic Registration

Any student that is 19 years old or younger on August 15th of the starting school year that is living with their legal guardian within the school district boundary and has passed five graded classes in their previous high school semester is assumed eligible for participation.

Out of state transfer students new to the district meeting the above criteria are also assumed eligible for participation. Students will need to complete an OSAA Eligible Student Transfer Certificate with their school enrollment. A transcript or grade card will be necessary to verify academic standing in the previous semester.

Students that enroll after the start of the school year will need to have an OSAA Eligibility Request Form evaluated by the Far West League Hardship Committee.

OSAA School Sports Pre-Participation Examination – Physicals are good for two years from date of issue. Previous physicals are on file in the Athletic Director's office. Typically incoming 9th grade, 11th grade and transfer students will need a physical exam.

School Insurance – Students must show proof of insurance to participate in athletics. In the event that the student is not covered under a current insurance policy, individual student insurance is available specific to school and sports participation. Enrollment forms are available at the Siuslaw High School office or at www.k12StudentInsurance.com.

When all elements of a student's athletic eligibility have been gathered, signed and presented to the Athletic Director's office, an athlete will receive a Coaches Eligibility Notice to present to their coach to show that they are now ready for participation.
