BEFORE your student-athlete can receive any equipment, or take place in any conditioning drills or practices, he or she must have a pre-participation physical on file in the Athletic Office.
This is not just an FHS rule, this is a requirement of the Michigan High School Athletic Association.
PHYSICALS (cont.) :
According to the MHSAA, to be valid for the current school year (August-June), the physical must have been performed on or after April 15 of the previous school year. The physical must be signed by a Physician (MD or DO), a Physician's Assistant (PA-C) or a Nurse Practitioner, and contain a statement certifying that the student has passed a physical examination and is physically able to compete in athletic practices and games.
All athletes should carry their own health insurance.
Optional insurance is available for your purchase. It is a great idea if you do not have health insurance, or you feel you need additional coverage.
Please note: The Fraser Public School District DOES NOT pay for medical or ambulance bills.
$75 for High School participants
The following information is a brief outline of the Athletic Activity Fee Program. The entire text of the policy is available in the Athletic Office.
1) Payment must be made prior to the season beginning for the following sports:
-Fall Season : Cross Country, Football, Girls Golf, Fraser Dance Team, Girls Swimming, Boys Tennis, Boys Soccer and Girls Volleyball
-Winter Season: Boys Swimming, Wrestling, Boys Basketball, Girls Basketball, Competitive Cheer, and Gymnastics
-Spring Season: Girls Tennis, Track & Field, Baseball, Boys Golf, Girls Soccer and Girls Softball
If payment has not been made by the first game of the season, the Student Athlete will not be allowed to practice or participate in games until payment has been received by the Athletic Office. Contact the Athletic Office if you are having difficulty in making the payment.
