The following online form is required prior to participating in the first scheduled practice for fall sports season. The OSAA Pre-Participation Sports Physical Form will still need to be completed and dropped off to the athletic office (if there is not a current form on file). This form can be downloaded from the link section found on the righthand side of this form. Physical forms can also be uploaded onto this application during the registration process. Physicals are required every 2 years (preferably Freshman & Junior years), and must be submitted on the OSAA physical form.
Students must be academically eligible and on track to graduate based on the criteria set forth by the OSAA-Schools Activities Association to participate in Athletics. Students not meeting these minimum criteria based on unique circumstances would need to meet with the Athletic Director Doug Thompson to discuss options.
Athletic Participation Fees - are required before approved to practice and/or tryouts begin. Payment should be taken to the bookkeeping office (located in the Main Office).
- $125 per sport (Gr. 9-12)
- $75 per sport (if qualify for reduced lunch, must provide confirmation letter for discount)
- $40 per sport (if qualify for free lunch, must provide confirmation letter for discount)
- Family cap of $400 (Activities not included in the Family cap - Cheerleading, Dance Team, Equestrian Team, Waterpolo, Snowboarding/Ski, Lacrosse)
