2018-2019 Athletics Registration

Participation

No student shall represent their school in interscholastic athletics until this registration form is on file with the Athletic Department as well as a signed physical form certifying that he/she has passed an adequate physical examination within the past year, noting that in the opinion of the examining physician, physician's assistant, nurse practitioner or a certified/registered chiropractor, is physically fit to participate in high school athletics; that student has the consent of his/her parents or legal guardian to participate; and, the parent and participant have read, understand and agree to the CHSAA guidelines for eligibility.

FEES

An Athletic fee of $100.00 is also required in full before any athlete can participate in practice/tryouts as well as all student fees paid in full per District 27J Policy. Athletic Fees can be paid online on the district website. If paying with Check or Cash please bring payment into the Athletic Office along with a completed current Physical Form. Payment can also be completed with a credit card through infinite campus. If payment is made through infinite campus please notify the Athletic Office as payment does not automatically transfer to Family ID.

All student fees (class, library, etc.) must be paid in full by the registration deadline.  Free and Reduced lunch waivers- Forms are available for qualification through the Nutrition Service Department or click on the https://sd27j.sdms2.com link to your right. Any questions, you may contact Doreen Davis, Athletic Secretary, 720.685.5408. Student athletes who qualify for F & R Lunch will have $75 waived. All athletes are responsible for $25.

ELIGIBILITY FOR PARTICIPATION Student athletes must maintain good grades. Student athletes must be passing all classes with at least a D to participate in games. Coaches will review athletic eligibility on a weekly basis. Please refer to the eligibility flow chart for more information. Referrals - Students who receive a referral will not be able to participate in the next sporting event.  Students must attend school for at least a half day to participate in sports. 

Coaches and the athletic director may deem a student ineligible at their discretion based on behavior on the playing field and in the classroom. PHYSICAL FORMS Please obtain from your doctor.

HOMESCHOOL/ONLINE/ATTENDING ANOTHER SCHOOL

When turning in your athletic paperwork, please include your last semester grades, contact information for attendance confirmation and proof of residency. (Note: your address of residency will determine the school you should attend for athletics.)
