No student shall represent their school in interschool athletics until this registration form is on file with the Athletic Department as well as a signed physical form certifying that he/she has passed an adequate physical examination within the past year, noting that in the opinion of the examining physician, physician's assistant, nurse practitioner or a certified/registered chiropractor, is physically fit to participate in high school athletics; that student has the consent of his/her parents or legal guardian to participate; and, the parent and participant have read, understand and agree to the CHSAA guidelines for eligibility.
An Athletic fee of $135.00 ($195.00 for non SD27J kids) is also required in full before any athlete can participate in practice/tryouts, as well as all student fees paid in full per District 27J Policy. Athletic fees can be paid online on the district website ($ Student fee payments tab). If paying with Check/Cash please bring payment into the Finance Office.
**NOTE: There will be a 24 hour window before you are cleared to participate in a practice.**
FEE WAIVERS (FREE OR REDUCED LUNCH)

If you are a transfer student from another high school, played sports for another high school, or an exchange student you will be required to notify the athletic office and to fill out additional paperwork and turn into the athletic office. Please see CHSAA links to your right.
HOMESCHOOL/ONLINE/ATTENDING ANOTHER SCHOOL
When turning in your athletic paperwork, please include your last semester grades, contact information for attendance confirmation and proof of residency. (Note: your address of residency will determine the school you should attend for athletics.) Please see CHSAA Form 6 link to your right.
ELIGIBILITY FOR PARTICIPATION
CHSAA requires that every student-athlete meet the minimum academic requirements in order to participate in high school athletics.
Students wishing to participate are required to meet standards of personal behavior and academic performance which are related to school purposes.
District 27J student-athletes may have only (1) failing grade and must be passing a minimum of (5) classes to maintain eligibility. Students who are not passing a minimum of (5) classes or have more than (1) failing grade (2- D’s equal 1 F) may not participate for the eligibility time period--weekly, quarterly or semester dependent upon the time frame and CHSAA requirements.
