2017 -2018 Athletic Registration

CLEARANCE

1. Before a student may try out or participate in any sport, cheer/song, dance team or band/color guard activity, this athletic registration must be read, filled out and signed, where appropriate, by his/her parent. This form contains pertinent information in regards to the athletic program and the student/parent responsibilities. This form must be filled out for each school year.


2. A physical and a medical doctor's clearance are required before any participation in any form of athletics, cheer/song, dance team, and band/ color guard, including tryouts. The official sports pre-participation physical form is the only form that will be accepted and can be found on the Yorba Linda High School website under the "Athletics" tab. All physical information including the Doctor's signature, license number, doctor's office address stamp and the box marked "Full contact collision level" must be marked. No Xerox or faxed copies will be accepted. The student health history must be completed, signed and dated by both parent and student. A physical is good for 1 full year. A new physical must be on file with the Athletic Clerk in the front office (do not give a coach or teacher any paperwork) in order to participate in any sport, cheer, dance or band.


3. A student must have medical and hospital insurance in order to tryout and/orparticipate in any sport, cheer/song, dance team or band/color guard activity . This is required by state law. Proof of insurance is required in the form of a copy of the front and back of your insurance card. If you do not have insurance you can purchase it by enrolling at www.myers-stevens.com. click the orange "Enroll Now" button on the home page, complete the enrollment process and your ID card will be emailed to you immediately.


4. The purchase of an ASB card for the year of participation is recommended. This is indicated on the student's ID card. Fall sport athletes clearing the previous spring/summer should purchase their ASB at Registration Week in August.
