All students who participate on an athletic team are charged an activity fee each season.
There will be a mandatory meeting for winter athletes and parents/guardians who did not attend the fall sports session with the Athletic Director on Monday November 20th at 6:45 in the Bow High School auditorium. Our meet the coaches session will take place that night at 7:15 in assorted classrooms.
The parents/guardians are responsible for filling out this form completely and ensuring that any and all medical information concerning your child's health, medical concerns, injuries, medications and allergies are reported to the Athletic Trainer & coaches.
The Bow High School Student-Athlete Parent Handbook for 2017-18 can be found under LINKS. The parent/guardian and the student athlete are expected to read this document so that they understand the rules of Bow High School and the NHIAA for participation in our athletic programs. The parent/guardian will be expected to register the student athlete and sign the agreements.
The electronic signature verifies that both the parent/guardian and the student athlete understand and agree to all policies.
Once Again, this form must be filled out by a parent/guardian of the student athlete!
The following information MUST be completed before a student athlete is allowed to participate.
• Parent/guardian and the student-athlete must review the information provided on concussion management, and are encouraged to view the free CDC concussion education course available in the main page LINKS section.
-Students new to Bow High School Athletics must submit a current pre-participation physical exam and a New Athlete Health History prior to any team tryout or participation in team activities or conditioning. Required forms are available in the LINKS section
-Any athletes who are unable to start the season at 100% due to injury or illness are required to provide a current doctor note specifying the nature of any limitations to participation.
