In order to be eligible to play sports at East Valley Middle School, you will need to complete this form and pay fees to the main office. When all of the requirements have been met, you will be “eligible" to turn out.
Have passed a physical examination from a medical authority approved by law. The current physical must be signed and is good for one year (13 months) from the date the physical took place.

Medical emergency information, parent consent and updated insurance information must be completed and signed during each registration period. Insurance coverage must include the minimum provisions listed on the parent consent form.
I understand that my son/daughter cannot participate in after school athletics unless the School Accident Coverage Plan or another plan covers him with a minimum payment for any injury of $25,000.
