The Middletown Area School District is committed to offering our students a wide variety of athletic opportunities that are designed to be an extension of the total educational development process of our students. The District is a member of the Pennsylvania Interscholastic Athletic Association (PIAA) and the Mid Penn Conference. At no time are these activities to assume priority over, or unreasonably interfere with, the objective of educational and curricular program. The success of our programs depends upon each individual developing himself or herself to the fullest extent possible and accepting responsibility to the team, thereby learning the valuable lessons associated with teamwork and doing one's best.
Sports Physical: Sections 5 and 6 of the PIAA Sports Physical posted below must be submitted to Mr. Rich Reed, Athletic Director. Section 6 must be completed and signed by an approved provider. Athletes who did not attend a free PIAA physical offered by the District over the summer, will be required to see their own provider for a PIAA physical.
Athletes who attend the free PIAA physical offered by the District will need to submit completed sections 1 and 5 to the physician onsite as they will complete section 6 during the exam.
- Athletes joining a sport more than 6 weeks after their initial physical exam, may be asked to complete the PIAA section 7 recertification form.
- Athletes who have sustained an injury or illness requiring medical attention, or had changes to their medical history at any point after the initial physical, may be asked to have their physician complete PIAA section 8 in addition to returning section 7.
- All athletes must complete the Extra-Curricular Participation Contract, Parental Code of Conduct and Expectations, and an Athletic Emergency form through the FamilyID system for each season of participation.
