The process in which student-athletes will submit their PIAA Comprehensive Initial Pre-Participation Physical Examination (Athletic Physical Packet) and supporting information has been changed for the upcoming school year. We are excited to announce that we are now offering the convenience of online registration through FamilyID.
FamilyID is a secure registration platform that provides students and families with an easy, user-friendly way to register for all athletic programming. When you register through FamilyID, the system records and stores your information in a FamilyID profile. Regardless of the number of athletic teams on which the student-athlete participates, you'll only need to enter the information one time.
All of the information which previously required handwritten responses can now be entered online (with the exception of Section 6 - the Medical Examination Form). Section 6 will remain a hard copy that student-athletes must have completed by a physician. Upon completion of the physical examination, you'll be able to upload Section 6 into your FamilyID account.
Please plan to complete your entire registration in FamilyID (including uploading PIAA Section 6) by August 1, 2020. You can scan or take a picture of the PIAA Section 6 to upload into FamilyID. When your registration is complete, you will receive two email notifications:
1 - When the registration has been RECEIVED
2 - When your entire registration process has been APPROVED
All late registrations, or those not receiving an approval email, will not be guaranteed the opportunity to participate on the first day of practice. You can always check the status of a student-athlete's Athletic Participation Clearance Status by logging into your FamilyID account and selecting "VIEW OR EDIT MY REGISTRATIONS." Select the "SUMMARY" tab at the end of the current registrations.
