Elkhart High School

     The Elkhart High School Athletic Department would like to welcome our student athletes and their families.  The 2020-2021 school year will be exciting as Elkhart Central and Memorial High Schools come together as one.  The Elkhart Community has a rich history of athletic accomplishments and achievement on many levels.  This tradition of success will continue to grow as we work together to create an atmosphere of engagement, encouragement  and sportsmanship at the new ELKHART HIGH SCHOOL!

     We are excited to work with FamilyID for athletic participation registration.   The following documents are required to participate in high school athletics per the IHSAA and/or Elkhart Community Schools.

1. IHSAA Preparticipation Physical Form

2. Concussion/Cardiac Arrest Form

3. Proof of Insurance Form

4. Drug Waiver Form

5. $40.00(per sport) Athlete Participation Fee

                         2020/2021 Athletic Physical Policy Update/Waiver


The IHSAA released the following information on April 22nd with respect to 20-21 Athletic Physicals:

 “Due to the COVID-19 pandemic during the 2019-20 school year and the continued stress on the medical community, the Indiana High School Athletic Association, Inc. will waive the provisions of Rule 3-10 for the 2020-21 school year.  Effective immediately, valid physical and consent forms presented to member schools on behalf of eligible students during the 2019-20 school year shall be valid during the upcoming 2020-21 school year.”

“Any athlete not having a valid IHSAA pre-participation examination physical form on file from the 2019-2020 scholastic year is required to have a valid Pre-Participation Physical and Consent Form completed prior to eligibility and competition. This includes any student-athlete coming from an out-of State school to an IHSAA member school or any student who did not participate in sports during the 2019-2020 school year. The student must have a valid Pre-Participation Physical and Consent Form completed on the official IHSAA form prior to eligibility.”

* If your child did NOT submit an IHSAA Physical Form for 19-20,  he/she will need a new physical for the 20-21 school year. 

**  Any student transferring to EHS from another Indiana High School may provide a copy of his/her 19-20 IHSAA Physical and Consent form.
